Responsibilities:
Responsible for providing appropriate Post-Acute Care and continuing care to all patients.
The Physician is a partner in an interdisciplinary team of post-acute care and administrative support staff to perform assigned duties while displaying the highest of standards of ethical and moral conduct as well as acting in the best interest, supporting our mission, vision and values to optimize the health and well-being of our patients.

Qualifications:

licensed to practice in the State of New York possessing an unrestricted DEA certification while maintaining required staff privileges at Garnet Health Medical Center and Garnet Health Medical Center- Catskills.
Minimum Education:
Degree of M.
D.
or D.
O.
with recognized internal medicine residency (3 years minimum) or ABFM-recognized family medicine residency (3 years minimum) required Minimum
Experience:
Professional clinical experience of 5 years in a Post-Acute Care setting preferred.
Required Certification/Registration:
NYS Medical License, valid and unrestricted required.
Board Certification or Board Eligibility in Family Medicine or Internal Medicine (within 5 years of training completion) BLS certification preferred.
Current Opioid Prescriber Training Certificate required.
Current Infectious Certificate required.
Physical Requirements:
Must possess the physical and mental abilities to perform the tasks normally associated with the practice of Post-Acute Service.

Estimated Salary: $20 to $28 per hour based on qualifications.
